Position Details

Facilitates entrance of new patients into care programs. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.

    \n
  • Responsible for the coordination of pre and post hospital care.
  • \n
  • Serves as the liaison between the patient and caregivers.
  • \n
  • Coordinates appropriate multidisciplinary clinics.
  • \n
  • Performs duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the multi-disciplinary program.
  • \n
  • Facilitates entrance of new patients into care programs.
  • \n
  • Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.
  • \n
  • Provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.
  • \n
  • Collaborates with other healthcare providers in coordinating and implementing the patient's plan of care including evaluation, treatment, follow-up and surveillance.
  • \n
  • Communicates with patient via mail, telephone, and in person regarding information pertinent to their care Coordinates patient and interdisciplinary activities to meet the needs of a defined group of patients.
  • \n
  • Acts as an effective resource.
  • \n
  • Coordinat's patient activities through the interdisciplinary clinics as appropriate.
  • \n
  • Reinforces patient and family education based on individual needs either in person or by telephone.
  • \n
  • Coordinates patient activities through available support services, such as support groups and social services as appropriate.
  • \n
  • Maintains systems and processes to assure timely follow-up and surveillance of post-follow-up patients.
  • \n
  • Provides nursing care and assists physician and procedures.
  • \n
  • Educates other team members in area of expertise.
  • \n

Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree-Nursing (Preferred)
